What are the differences between biostimulators and classic fillers?

Dr. Janine Hopkins, a dermatologist in Southlake, TX and Monroe, LA, explains that the primary difference between biostimulators and dermal fillers lies in how they restore volume and rejuvenate the skin. Hyaluronic acid-based fillers add volume instantly by physically filling areas under the skin, making them ideal for smoothing wrinkles and enhancing facial contours. On the other hand, biostimulators like Sculptra stimulate the body’s natural collagen production over time, resulting in improved skin thickness, elasticity, and structure. The effects of biostimulators develop gradually and can last up to two years or more. There are also hybrid options like Radiesse, which provide immediate results along with biostimulation.

It’s important to note that while hyaluronic acid fillers can be dissolved, biostimulators cannot, as mentioned by Dr. Tina Alster, a dermatologist in Washington, D.C.

Classic fillers 101

Classic fillers such as Juvederm and Restylane, which are made of hyaluronic acid, are injected into the skin to create immediate volume, explains Dr. Alster. While the results are visible faster, they also tend to fade quicker compared to biostimulators, lasting between six to 18 months.

Biostimulators 101

Biostimulators are unique in that they contain materials designed to stimulate the body to produce new collagen independently, states Dr. Alster. This leads to improvements in both volume and skin quality. A series of treatments is typically recommended, with results becoming noticeable after three months or more, but lasting longer.

While hyaluronic acid fillers provide immediate results by filling in lost volume, biostimulatory fillers like poly-L-lactic acid offer gradual results by stimulating collagen production over time, as explained by Dr. Jennifer Segal, a dermatologist in Houston, TX.

When to choose a classic hyaluronic acid filler

One of the main reasons to opt for a hyaluronic acid filler is for immediate volume enhancement. It is also suitable for smaller areas such as the lips and under eyes, notes Dr. Segal. Dr. Hopkins recommends fillers when quick results are desired, such as plumper lips or enhanced cheekbones. Hyaluronic acid fillers are also recommended for beginners due to their shorter duration and ability to be dissolved if needed for patient safety.

When to choose a biostimulatory filler

If you’re looking for subtle volume enhancement and skin quality improvement, a biostimulator may be the best choice. They are preferred by patients seeking a gradual and natural-looking transformation. Dr. Segal suggests using biostimulators for larger areas like the temples, as they can be more cost-effective in the long run.

A case for a combination

Instead of choosing between hyaluronic acid fillers and biostimulators, some doctors recommend a combination approach for optimal results. Dr. Hopkins advocates for strategic layering of both fillers to achieve immediate enhancement and long-term collagen remodeling. Dr. Hartman also supports using a combination of fillers for a harmonious and natural outcome.
